Attendance Procedures

  • If your student will be absent from school, late, or leaving early please call 650-738-6687 for the main office. Leave your student’s name, teacher, grade, and reason for absence.

School Volunteer Documents

Pacifica School District has adopted updated volunteer requirements to align with state law and ensure consistency across all schools. These requirements will apply to volunteers working directly with students during the school day or attending field trips. 

Schools will support volunteers through this transition period, and enforcement of the new procedures will be gradual to ensure adequate time for completion.

Paperwork Needed by Role

Classroom and General School Volunteers

  1. Volunteer Application Form (once a year)
  2. Current Proof of Negative TB Test (every 4 years)
    • You can submit current proof of TB via any one of the following methods:
      1. TB Assessment Form signed by a medical professional – CA School Tuberculosis (TB) Form OR
      2. Proof of Negative TB from a medical professional OR
      3. ​​Cleared TB Chest X-Ray
  3. Proof of Valid Photo Identification (current)
  4. California LiveScan Application (once as long as you have a child in PSD)
    • can be taken to any UPS store (such as in Daly City), and processed for a cost ($65-80). Download form for fillable fields.
      • If you have already had the LiveScan processed, please send a copy with the ATI number completed by the Live Scan Operator.
      • You can also check your status using said ATI number.
      •  If you need help covering the cost of live scan, contact treasurer@osspto.org.

You can also be cleared as a volunteer through Pacifica School Volunteers.

Reimbursement / Tax Deductible Requests

​Have you bought supplies for a class or the school? You may be eligible for a Reimbursement or Tax Deductible Receipt. Please submit a completed Reimbursement / Tax Deductible Request Form​ with receipt(s) to checks@osspto.org OR paper copies can be turned into the PTO box  at the start of the A-wing hallway.
